Linking Exercise Limitations to Muscle Abnormalities in ME/CFS Using MRI and NIRS

Many people with ME/CFS experience reduced exercise capacity, muscle weakness, and post-exertional malaise (PEM). However, it is still unclear exactly what happens in the muscles. Previous research suggests that some people with ME/CFS have reduced oxygen delivery to their muscles, while others extract less oxygen from the blood.

Aim and Approach
The project investigates which changes in the muscles are associated with exercise limitations and PEM. Researchers also aim to develop patient-friendly measurement methods that can be used in both research and clinical care.
The project consists of two sub-studies. In collaboration with Harvard Medical School, researchers will re-examine muscle samples and exercise tests to gain a better understanding of muscle changes. In the Netherlands, 80 people with ME/CFS and 20 healthy volunteers will be studied using MRI, MRS, and NIRS. This will allow researchers to assess muscle structure, oxygen use, and energy production without placing additional strain on the body.

Expected Results
The researchers hope to gain a better understanding of why physical exertion leads to prolonged exhaustion in people with ME/CFS. The findings will help develop reliable, non-invasive measurement methods and could contribute to personalised treatments in the future.
